• Central Registration for New Students and Returning Students

Welcome to the Dunkirk City School District.  We look forward to working with you as a member of our school community.  All new and returning students are registered through our centralized registration process.  Registrations are done by appointment and the parent/guardian of the student must come to the office to complete the registration.

Here is a list of the information that you will need to bring to the appointment:

    • Proof of residency in the district

    • Birth Certificate for each child

    • ID for the parent/guardian

    • Proof of guardianship, if applicable

    • Immunization Record for each child

Other helpful information to bring may include:

    • Name, address, phone number and fax number of previous schools attended

    • Previous report cards and progress reports

    • IEP’s

    • 504 Plans

During the registration appointment you will be asked to give information that will be recorded on Dunkirk City School District registration forms. These forms include the registration form, emergency contact form, residency questionnaire, school lunch application, consent to use school computers, home language questionnaire, and racial/ethnic identification form. You will also be asked to sign a Consent for Release of Information form so that the district can request information from previously attended schools.
